Consider excluding Patients transferred to another health care facility

      Thank you for your question specific to CMS506v4 (Safe Use of Opioids – Concurrent Prescribing). The measure currently excludes patients discharged to another acute facility (Discharge to Acute Facility- 2.16.840.1.113883.3.117.1.7.1.87), however you are correct that it does not exclude patients discharged to other types of health care facilities. We appreciate the feedback that other facilities may be similar to acute care in that medications are dispensed by clinicians and patient reactions are closely monitored. We will consider your suggestion to exclude patients discharged to health care facilities not covered under the Discharge to Acute Facility value set during the eCQM Annual Update.​

    • Patients with med orders provided as part of the transfer process to another health care facility (Swing Bed / Rehab / LTAC) are being counted in the numerator for this measure.

      Patients transferred to another health care facility (Swing Bed / Rehab / LTAC) can have pended med orders from the transferring provider to improve the transfer of care. Those med orders are then reviewed by the receiving provider.  It does not seem like these patients should be part of the intended population for this measure and should be excluded.
