      Thank you for your question about the Numerator for CMS146v10 “Appropriate Testing for Pharyngitis.” The intent of the measure's Numerator is to look for a group A streptococcus test in the seven-day period from three days prior to the episode date through three days after the episode date. The logic performance between the 2021 and 2022 spec are not performing the same. For the 2022 specification, we did find an issue where the timing is not counting date level precision 3 days before the encounter as we expected. We are investigating the issue and will propose a solution during this upcoming annual update.

    • Not sure if the Strep test "within 3 days" includes both before AND after Pharyngitis encounter end. Need clarification for 2022 configuration guidance.

      While reviewing the espec for CMS 146 version 10 Numerator for 2022, we noticed the wording had changed slightly from last year:

      2021
      from
      "Group A Streptococcus Lab Test With Result" GroupAStreptococcusTest,
      "Encounter With Pharyngitis or Tonsillitis" EncounterWithPharyngitis
      where ( GroupAStreptococcusTest.relevantDatetime 3 days or less on or before day of
      end of EncounterWithPharyngitis.VisitWithAntibiotic.relevantPeriod

      or GroupAStreptococcusTest.relevantDatetime 3 days or less on or after day of
      end of EncounterWithPharyngitis.VisitWithAntibiotic.relevantPeriod

      )
      return EncounterWithPharyngitis.VisitWithAntibiotic

      **2022 **
      from
      "Group A Streptococcus Lab Test With Result" GroupAStreptococcusTest,
      "Encounter With Pharyngitis or Tonsillitis With Antibiotic" EncounterWithPharyngitis
      where ( Global."NormalizeInterval" ( GroupAStreptococcusTest.relevantDatetime, GroupAStreptococcusTest.relevantPeriod ) starts within 3 days of
      end of EncounterWithPharyngitis.relevantPeriod

      )
      return EncounterWithPharyngitis

      We wanted to clarify and confirm that the wording means the same thing from 2021 year to 2022 year and that the configuration should not change here as far as that highlighted timing value goes.

      Please clarify and confirm. Thank you for your help in advance.
