Thank you for your question. Since the 2021 version of the measure (506v3) does not specify that expired patients should be excluded, we cannot give guidance that these patients should be excluded from the denominator. However, you are correct that the measure intent is to capture patients who are alive at the time of discharge. We recommend updating your workflow to automatically discontinue all medications upon discharge for expired patients as a workaround for 2021. This will prevent patients from being captured in the initial patient population.

    • Inaccurate data being sent/reported.

As we are validating our data for the Safe Use Opioids - Concurrent Prescribing measure (CMS 506) we've come across patients who are expired and being applied to the numerator.  In further investigation it looks like these patients still have active prescriptions that have NOT been discontinued at discharge.  As we know, these patients are not continuing on these prescriptions at discharge.  I saw the tickets CQM-4478 & CQM-4192 reference this issue, but not that these patients were falling into the numerator for them.

For reporting CY 2021 reporting we are wondering if it would be appropriate to exclude these patients from our data since the CY 2022 logic indicates these will be in the denominator exclusion bucket?
