Good afternoon, 

We are seeking clarification on objective metric "Support Electronic Referral Loops by Sending Health Information" for eligible hospitals (Promoting Interoperability Program)

The document on the CMS PI portal states: "(TOC) At a minimum, this includes all discharges from the inpatient department and after admissions to the emergency department when follow-up care is ordered by an authorized provider of the hospital". Should we interpret this as:

• All inpatient discharges with follow-up care ordered

          AND

• All emergency department admissions with follow-up care ordered

Or as:

• All inpatient discharges (disregarding whether follow-up care was ordered or not)

          AND

• All emergency department admissions with follow-up care ordered
