Logic and value sets were updated in the annual update published June 17, 2013 to include detection and interventions for BMI outside of normal limits.
The Guidance in the header for eCQM 69 states, "The documented follow up interventions must be related to the BMI outside of normal parameters, example: “Patient referred to nutrition counseling for BMI above normal parameters”."
The Numerator Logic in the Population Criteria does not seem to directly require the relationship as stated above. In the example below we are looking at the most recent BMI <= 6 month(s) before or during Occurrence A of Encounter and if the BMI is outside of normal parameters (<23 KG/m2), a follow up plan (intervention or medication order)must occur <= 6 month(s) before or during the same encounter. There is no requirement that the BMI outside of normal parameters be documented at the same time as the follow up plan, only that they both occur during the encounter or 6 months prior which would allow for those 2 elements to be documented at different times.
OR:
AND: MOST RECENT:"Occurrence A of Physical Exam, Finding: BMI Finding Code Set" <= 6 month(s) starts before or during "Occurrence A of Encounter, Performed: BMI Encounter Code Set"
AND: "Occurrence A of Physical Exam, Finding: BMI Finding Code Set (result < 23 kg/m2)"
AND:
OR: "Intervention, Order: Below Normal Follow up" <= 6 month(s) starts before or during "Occurrence A of Encounter, Performed: BMI Encounter Code Set"
OR: "Intervention, Order: Below Normal Referrals" <= 6 month(s) starts before or during "Occurrence A of Encounter, Performed: BMI Encounter Code Set"
OR: "Medication, Order: Below Normal Medications" <= 6 month(s) starts before or during "Occurrence A of Encounter, Performed: BMI Encounter Code Set"
Can you provide some guidance as to what is actually required for reporting?
