Based on the measure guidance, the follow-up plan documented must be related to a positive depression screening. Per the measure specification, the " Follow Up for Adolescent Depression" (2.16.840.1.113883.3.526.3.1569) and "Follow Up for Adult Depression" (2.16.840.1.113883.3.526.3.1568) value sets include codes that identify acceptable follow-up plans that would meet numerator criteria. You are correct that the guidance provides a limited list of examples, and these examples reflect a subset of all of the options found in the value sets for follow-up plans for adolescent and adult depression. Note that the definition section states the following about the minimum requirements for a follow-up plan:
Documented follow-up for a positive depression screening must include one or more of the following:
* Referral to a practitioner who is qualified to diagnose and treat depression
* Pharmacological interventions
* Other interventions or follow-up for the diagnosis or treatment of depression
While the codes within these value sets include codes for mental health care education (procedure) and mental health promotion education (procedure), none of the codes reflect that simply giving the patient depression specific print material is acceptable. Giving the patient only a handout also does not meet the definition of what must minimally be included in a follow-up plan.
