Confirmance on Value Set related to "Intervention, Performed not done: Medical Reason" for Tobacco Use Cessation Counseling

      The Medical Reason value set is a standard value set used across many EP measures; the measure developer acknowledges that not all applicable SNOMED concepts will apply in every situation, but a decision was made to develop a standard list that would be applicable in most instances.

      We are unsure of what question is being asked with the first statement included in the JIRA issue: " In context of NQF # 0028-Preventive Care and Screening: Tobacco Use: Screening and Cessation Intervention- Tobacco Use Cessation Counseling value set (2.16.840.1.113883.3.526.3.509) includes course of action without any direct physical manipulation (education, referrals, etc.) and Tobacco Use Cessation Pharmacotherapy value set (2.16.840.1.113883.3.526.3.1190) includes specific medications."

      Regarding the exceptions, please review the updated eMeasure, posted June 14, 2013, as the denominator exception construct has been updated. The medical reason exception now only includes: "Risk Category Assessment not done: Medical Reason" for "Tobacco Use Screening Grouping Value Set" during "Measurement Period" so the list of SNOMED concepts included in the "Medical Reason" value set may be applicable reasons for not screening the patient for their tobacco status.

      Also both the below denominator exception criteria refers to same value set (2.16.840.1.113883.3.526.3.1007)that includes all medical reasons for negation of clinical concept:
      1. Intervention, Performed not done: Medical Reason" for "Tobacco Use Cessation Counseling Grouping Value Set
      2. Medication, Order not done: Medical Reason" for "Tobacco Use Cessation Pharmacotherapy Grouping Value Set

      Medical reasons specified for the negation of tobacco use cessation pharmacotherapy seems to be relevant (e.g. a particular tobacco use cessation medication may not be given because of drug resistance/allergy). However, are the medical reasons (as defined by value set) for negation of cessation counseling also relevant? How cessation counselling service (smoking effects education, referrals) cannot be given because of medical reasons such as History of - drug allergy, Drug resistance, Medical contraindication, etc?
