The logic is structured in a way that allows for tracking of changes in functional status over time. In 2022 CMS 90 (CMS90V11), if a patient with heart failure is seen not seen in the first half of the measurement period (180 days or more before end of measurement period), but is seen twice in the second half of the measurement period, they automatically fail the measure. The patient gets into the initial population, but there is no way to to complete the numerator.
