      Thank you for your question about CMS506v3 "Safe Use of Opioids - Concurrent Prescribing." Yes, to satisfy the numerator, a patient would need two distinct opioid codes from the valueset Schedule II & III Opioid Medications (2.16.840.1.113762.1.4.1111.165) OR one opioid code from Schedule II & III Opioid Medications and one benzodiazepine code from Schedule IV Benzodiazepines (2.16.840.1.113762.1.4.1125.1). The date of service does not matter in the measure. The intent of the measure is to assess medication at discharge, meaning medication the patient will be taking directly after discharge. This includes 1) opioids/benzodiazepines the patient was prescribed before the inpatient encounter and will continue after discharge; 2) opioids/benzodiazepines the patient was prescribed during their inpatient stay and is expected to continue after discharge; and 3) opioids/benzodiazepines the patient is prescribed at the point of discharge and is expected to take post-discharge. All of these medications should be included in the QDM datatype Medication, Discharge. For additional information on Medication, Discharge, please see the eCQI Resource Center https://ecqi.healthit.gov/mcw/2021/qdm-dataelement/medicationdischarge.html. Please let us know if you have additional questions.

      Hi Team,

      Consider statements below from Numerator of CMS506v4:

      Numerator:
      /*Encounters of patients prescribed two or more opioids or an opioid and benzodiazepine at discharge.
      */
      ( "Inpatient Encounter with Age Greater than or Equal to 18" InpatientEncounter
      where ( Count(["Medication, Discharge": "Schedule II & III Opioid Medications"] Opioids
      where Opioids.authorDatetime during InpatientEncounter.relevantPeriod
      return distinct Opioids.code
      )>= 2
      )
      )

      Query:
      1. Should we be taking opioid code as two distinct codes from the mentioned valueset "Schedule II & III Opioid Medications" (2.16.840.1.113762.1.4.1111.165) in the measure document for satisfying the below statement. If yes, should the date of service associated with the distinct codes be same or distinct?

      Request you to provide with the resolution urgently as our client deliverables are dependent on it.
