      QUESTION 1: Regarding Denominator Exclusion– when is the EXACT diagnosis Start and Stop date for Depression and Bipolar disease in order to qualify for the Denominator Exclusion?
      The diagnosis must start before the encounter and must not end before the encounter (thus be an active diagnosis prior to the start of the encounter)

      QUESTION 2: Regarding Denominator Exclusion: is the exclusion criteria for Depression and Bipolar disease “ever”, i.e - anytime in a patient’s history? Are exclusions to the denominator designed to be ever?
