      NQF 0371 and 0372 specify subcutaneous or intravenous administration routes for unfractionated heparin and no route is identified for low molecular weight heparin. NQF 0376 specifies intravenous route for unfractionated heparin and subcutaneous route for low molecular weight heparin. IQR VTE-1,2,6 VTE Prophylaxis Inclusion Table (Appendix H, Table 2.1) specifies subcutaneous route for unfractionated heparin and no route is identified for low molecular weight heparin. Why are the VTE prophylaxis unfractionated heparin and low molecular weight heparin administration routes different between measures?
