QRDA Category III reporting patient level vs. encounter level

      A general question with respect to quality reporting - should a facility report unique patient counts or unique encounter counts using the QRDA Category III?

      If I am not wrong, CMS wants providers to capture and report demographic elements like Race, Ethnicity, Language etc. to gauge/analyze health conditions and diseases for different population sets. If the reporting is at an encounter level, a patient with multiple encounters will get counted more than once. Will that give an accurate picture to CMS?

      If the reporting is intended to happen at a patient level, a patient with multiple encounters might meet performance for a measure in one encounter and may not meet performance for another. Overall, should the patient be reported under performance met or performance not met for that measure?
