      You are correct that the Apixiban 5 mg dose pack associated with RxNORM code 1992427 is currently not in the 2019 reporting year Antithrombotic Therapy value set 2.16.840.1.113883.3.117.1.7.1.201. The RxNORM code has been added to the Antithrombotic Therapy value set for the 2020 reporting year, shown on the 05/10/2019 update.

      The only way you can successfully use this code is to perform custom mapping for this reporting year. This entails finding an equivalent value set in your EHR for the current reporting year.

      For the 2019 reporting year, we have patients who are not meeting the CMS 104 measure numerator because the physician has prescribed the apixiban dose pack (apixaban 5.0 mg tablets) with RxNorm code 1992427 at discharge. I understand that this RxNorm code has been added to the 5/10/19 version of value set 2.16.840.1.113883.3.117.1.7.1.201 which will be effective in the 2020 reporting year.
      For the 2019 reporting year, is there a way that we can include this apixiban starter pack in the numerator - possibly with a custom mapping to the apixiban 5 mg tablet with RxNorm 1364445 (which is in the 9/17/18 version of the value set)?
