Thank you for your feedback. You are correct; the Apixiban Dose pack should be included in both the Antithrombotic and Anticoagulant Value Sets. It was inadvertently excluded because it is listed as a GPCK- Generic Pack. Currently, the content model in the value set uses Semantic Clinical Drug. We will add this to the next annual update in 2019.

Starter Paks appear to use different RxNorm codes: For example, APIXABAN 5 mg (74 tabs) tablets in a Starter Dose Pack use RxNORM code 1992427 which is not included in the value set However, Apixaban 2.5 MG and 5.0 MG Oral Tabs use RxNORM code 1364445 which is included. In our reporting we discovered a patient that did not meet eCQM CMS 104 (STK-2) discharged on Antithrombotic therapy even though they were given an appropriate antithrombotic medication. Could you explain why the starter pack meds don't seem to be included in the value set? Could we request that they be included?
