    • What is the correct SNOMED code for the one value in this value set?
    • This is the correct identifier for the concept. SNOMED CT concepts can be up to 18 characters long. See Comment for more.

STK10 measure uses the value set "Discharged to Rehabilitation Facility SNOMED-CT Value Set (2.16.840.1.113883.3.117.1.7.1.132).

This value set in VSAC has one value -
433591000124103 Discharge to rehabilitation facility (procedure)

The version is indicated as Provisional. So I'm guessing this code has been requested to be added to SNOMED.

The issue is with the length of the code. Most SNOMED codes are up to 9 characters or digits. This code is 15 characters. Is this truly the code we need to use for this value? The code seems incorrect to me.

Any guidance on this would be very much appreciated.

      Thanks!
