      Thank you for your question. The measure is looking for Low Dose Unfractionated Heparin for VTE Prophylaxis, not for VTE treatment. The Low Dose Unfractionated Heparin for VTE Prophylaxis value set (2.16.840.1.113762.1.4.1045.39) includes only concentrations used for prophylaxis. RXNORM code 1659195, 500 ML heparin sodium, porcine 50 UNT/ML Injection, you reference is used for VTE treatment, and is therefore not included in the value sets for this specific measure. Your question also states the patient is being treated for a PE “We have a case with IV heparin 25,000 units in dextrose 5% infusion PREMIX running continuously at 18 ml/hr for an acute PE.” The Initial Population statement and logic specifically does not include patients with a diagnosis of venous thromboembolism (VTE).
       
      Initial Population: Patients age 18 and older discharged from hospital inpatient acute care without a diagnosis of venous thromboembolism (VTE) or obstetrics with a length of stay less than or equal to 120 days that ends during the measurement period.

      Hello; this is related to the same case below (heparin premix for VTE treatment), where the case was in denominator and failed for VTE-2; however, I would like understand why the following two codes below were not in the value set for VTE to originally exclude the case. ICD T82.868A & I74 (patient had claudication and had a principle diagnosis of thrombus within a stent graft body (iliac). Thank you.

      Why is a heparin drip such as a premix in D5W not included as one of the RXNORM codes? It appears the existing RXNORM code 1659195 should apply here but we were told by our eCQM team that it is not. We have a case with IV heparin 25,000 units in dextrose 5% infusion PREMIX running continuously at 18 ml/hr for an acute PE. The above code is for 500 ml Heparin Sodium, porcine 50ml/UNT/ML injection. Therefore, 500 X 50 = 25,000. If it is not included can you please give a reason.
