Advanced illness includes end-stage of illnesses such as end-stage renal disease (ESRD), heart failure, neoplasm, etc. Frailty includes the use of devices such as wheelchairs, hospital beds, etc. The patient must meet both criteria. Proposed Solution: Add exclusion for patients with advanced illness and frailty because it is unlikely that patients need the services being measured. Rationale: In some cases, it might even be harmful for patients to focus on a particular service when there should be other priorities in their health care.
