We have several facilities using the same EHR system. Each facility has its own unique Pt ID number. We are seeing cases at facility 'A' passing the measure based on documentation at facility 'B'. Also, the same is occurring within the same facility, visit 'A' is pulling visit 'B' documentation. Has this issue been addressed in the measure logic, or is this an issue that our EHR vendor needs to fix? I'm not sure which version of criteria we currently have.
