      ​Thank you for your question. Currently, the eCQM does not have a stand-alone denominator exception that can cover this fallout. However, the eCQM does have a denominator exception for medical reason for not providing antithrombotic therapy. We will consider adding a stand-alone exception for elevated INR in a future version update.

    • Antithrombotic therapy not given by end of Hospital Day 2 due to elevated INR on admission is being captured as a fallout for this measure.
    • Patients with an elevated INR level (>3.5) are falling out of the measure. There is no code in the medical reason value set (2.16.840.1.113883.3.117.1.7.1.473) to exclude these patients.

      We are proposing to add a denominator exception to accout for an INR value greater than 3.5​. We are suggesting the following changes:

      1. Add INR value set (2.16.840.1.113883.3.117.1.7.1.213) to denominator exceptions. Description: This set of values contains codes for laboratory test for INR, or International Normalized Ratio. Inclusion criteria: Only includes codes which represent laboratory test of INR. These are LOINC codes.

      2. Proposed Logic in red:
