      Rework of this measure deferred to the update with particular attention to simplifying denominator exclusions. Likely some improvements would lead to improved ease in implementation.

      Note: EPIC mentions that whether the patient was in a nursing home in the past year is not a data element captured generally by EHRs, and that it would require asking the patient. Even after asking the patient, the confidence in this data would still be relatively low. ONC and CMS are aware of the limitations of this data element and will work with new measures to clarify in logic the capture of this type of data.

      This measure has nine different denominator exclusions, including a check of whether the patient had a pacemaker implanted within 4 days after a CABG, and a numerator which depends on whether the patient spent any time in a nursing care facility. A simple measure has become exponentially more complex. �And we have to ask ourselves, how will we know if the patient was in a nursing care facility and how will require the users to record that?
