Is general statement on after visit summary to follow-up with PCP if your doctor discussed your high-blood pressure with you today in compliance with the intent of the measure

      ​A summary advising the patient to schedule an appointment with their PCP for a follow up does not meet the intent of the measure. If a patient is pre-hypertensive or hypertensive during a physician visit, the physician can rescreen the blood pressure within the parameters set forth in the specification or make a referral to an alternate/primary care provider.

      Measure specifications for registry reporting indicate follow-up with PCP for high-blood pressure is acceptable follow-up. However, is it in compliance that a follow-up message is added to every after visit summary stating "If your provider discussed the need to manage your blood pressure, you should schedule an appointment with your primary care provider for a follow-up"
