Data accuracy for identifying patient problems
Currently, VSAC eCQM value sets contain SNOMED Concept Ids and many EHR's contain SNOMED Concept Id synonyms. This impacts the ability to capture structured data mapped to synonym SNOMED codes that are used in EHR Problem Lists. We are requesting SNOMED synonyms of the SNOMED Concept Ids to be added to eCQM value sets. Attached is a Diabetes mellitus type 1 screen shot from SNOMED CT (July 2017) as an example of the relationship of SNOMED Concept Ids to SNOMED synonyms.
