      Thank you for your question. Currently, the eCQM does not have a stand-alone denominator exception. However, the eCQM does have a denominator exception for medical reason for not providing antithrombotic therapy. We will consider adding a stand-alone exception for INR that is supratherapeutic in future versions.

    • Patients assigned "Not Met" when a patient has a supra-therapeutic INR on day two of hospitalization.

      Logic should be included to evaluate the patients INR day two – we feel it inappropriate to require providers to specifically document a reason (in a codified field) when the lab value resulted in the EMR should suffice.
