If a patient is receiving an actual dose of 0.7 mL, that medication should be mapped to one of the Enoxaparin Rx Norm codes found in the value set, as these are generalizable drug concept codes. If you believe this is in error, please provide the specific Rx Norm code you believe should be in the value set.

      The value set 2.16.840.1.113883.6.88 for stroke patients receiving antithrombotic therapy at discharge has listed 0.6 ml Enoxaparin (Lovenox) and 0.8 ml Enoxaparin 100mg/ml in the value set. Currently we are having patients fail as they are being prescribed 0.7 ml Enoxaparin at discharge. We have checked the latest version at the value set authority dated May 5, 2017, it is not present there.

      Are there plans from the measure steward to add additional dosages of Enoxaparin to meet this measure? This is not a work flow issue, but a patient safety issue.
