For CMS-108 and CMS-109, based on current value set mapping for unfractionated heparin, our patient's who received a heparin drip in the allotted time frame at our facility are inappropriately being counted as denominator patients. The Heparin Drip our facility uses for inpatients is compounded in our hospital pharmacy using heparin 10000 unit/mL and dextrose. The value sets for CMS108 and CMS109 measures only allows for use of premix heparin drip rxnorm codes (e.g. 25000 unit/500 mL); thus, resulting in all the patients in our facility that receive a heparin drip are ending up in the denominator, but should be in the numerator since they did technically receive a heparin drip in appropriate the timeframe.
