Need Guidance on workflow and Mapping on issues with CCN's between hospitals and ED that share the same CCN

The Transfer concept captures patients who are transferred into your hospital from an outside hospital where they were an inpatient or outpatient or transferred in from any emergency department (ED) or observation unit OUTSIDE of your hospital. Regardless of the CCN, the intent is these patients are excluded because of having potentially received care prior to arrival to the receiving ED. Therefore, if the patient is "transferred" the patient would be excluded. But if the patient was discharged from your ED and admitted inpatient to your hospital in less than or equal 1 hour, he would be included.

    • Need answer for proper mapping and workflow

      We have several freestanding ED's and 2 hospitals that share the same CCN. I read the answers about the hospitals with the same CCNs and patients who spend time in both ED's (transfers from one ED to another) will have each ED encounter's show on the reports for both hospitals ED's under the same CCN.

      My question is on Hospitals and Freestanding ED's who share the same CCN when they see the patient in the freestanding ED and then transfers the patient as direct admit to the hospital when the hospital is in a very different location?
