In the scenario provided, provider A would be the only provider that receives numerator credit since this provider performed the depression screening. Provider B and C did not complete a depression screening, which does not satisfy the intent of the measure. A depression screening must occur on the date of the encounter, each provider should screen and provide follow-up care as appropriate for a positive screen. Once the patient has been identified as having a diagnosis of depression or bipolar disorder they are excluded from this measure.

For CMS 2, does this have to be provider specific? If a patient is seen on 1/14 by provider A and the screening for depression was done. The patient was then seen on 3/15 by provider B and 6/16 by provider C. Neither of these visits had a screening for depression and the patient was found to not be depressed. Should provider A be the only provider that gets numerator credit for this measure or should provider B and C get credit as well? Would this answer change if the screening was positive and provider A referred the patient out to see someone?
