In the context of “during”, what should be used to indicate the encounter end date/time, the date/time the patient is discharged from the facility or the date/time the discharge order was written? In other words, is the intent of the measure to assess documentation prior to the date/time the patient was discharged from the facility or the date/time the discharge order was written?
