After careful review and discussion with the steward, it was decided that due to vendor difficulty related to extraction of the diagnostic study result, modification of the logic would occur. The logic update occurred during the April 2013 update. The denominator logic related to “Diagnostic Study result: Hospital Chest x-ray/CT scan (result: ‘Hospital Measures-Radiologic findings consistent with pneumonia’)” was moved to a denominator exclusion. The new denominator exclusion logic is as follows: “Diagnostic Study result: Hospital Chest x-ray/CT scan (result: ‘Hospital Measures-Radiologic finding normal ’)”. Updated measure specifications are available on the CMS website.

In the majority of EHR systems integrated with or interfaced with imaging systems, the presence of abnormal findings on an x-ray or CT, let alone abnormal findings suggestive of pneumonia, are not recorded in a discrete fashion. Any documentation of these findings must therefore be done in a manual redundant workflow. Given this infeasibility, consideration should be given to eliminating this criterion and include these patients in the denominator without it.
