      We have a question about CMS 069, NQF 0421: Preventive Care and Screening: BMI screening and follow-up. The logic model says if the BMI is measured and documented in the chart then use intervention order code above (G8417), normal or below (G8418) normal follow-up or do referrals? Is that sufficient? Please advise. See image below for logic model.
