In the narrative section for the 2015 eCQM specification, it says "Patients with a principal diagnosis of ischemic stroke, history of atrial ablation, and current or history of atrial fibrillation/flutter", which is a mismatch of what the logic section says. This narrative statement should read: "Patients with a principal diagnosis of ischemic stroke, and a history of atrial ablation, or current or history of atrial fibrillation/flutter", and was corrected in the 2016 annual update cycle. The logic reflects this statement, so in order to satisfy the denominator, you need to have a principal diagnosis of ischemic stroke and either a history of atrial ablation or a current diagnosis or history of atrial fibrillation/flutter.
