Unattainable measure requirement

      This measure follows the CDC and ACIP guidelines for immunizations. We realize these vaccines are not mandatory. However, we do not allow patient refusal as an exclusion for these measures. We assume that this is an issue across all providers. Therefore, reporting rates are affected equally for all providers.

      We plan to add CVX45, CVX89, CVX17, CVX45, CVX109, CVX85, and CVX88 to their respective vaccination value sets. However, CVX122 (Rotavirus, unspecified formulation) will still be excluded from our measure. The Childhood Immunization Measure requires either a 'dose 2 schedule' or 'dose 3 schedule' for Rotavirus vaccine. CVX122 therefore is just not specific enough to meet our inclusion criteria.

      I work with CHRISTUS Health in Coushatta, LA. I am over the CQR Meaningful Use Measure program for our providers at our rural health clinics. The Special Registry for Immunizations has an unattainable measure requirement:

      Vaccinations:
      4 diphtheria, tetanus and acellular pertussis (DTaP)
      3 polio (IPV)
      1 measles, mumps and rubella (MMR)
      3 H influenza type B (HiB)
      3 hepatitis B (Hep B)
      1 chicken pox (VZV)
      4 pneumococcal conjugate (PCV)
      1 hepatitis A (Hep A)
      2-3 rotavirus (RV)
      2 influenza (flu) vaccines BY THEIR SECOND BIRTHDAY

      The issue we have is on several levels:

      The first issue is that the Flu vaccines are not mandatory immunizations and you cannot force a parent to have their infant and toddler receive the shots.

      The second issue is that the Hep A vaccine is also not a mandatory immunization.

      The third issue is that since we are a rural health clinic, we see patients that are from out of town and if that patient happens to have had a visit in this year and is 2 years of age or younger, then that child counts against us, even if they are from out of state.

      The fourth issue is that when we go and load a child’s shot record from the LA links database into our database, we can only see the manufacturer of the vaccine and not the exact name of the vaccine that was given. Since we can’t just choose a vaccine by name randomly due to different vaccination combinations, we have to choose unspecified formula. This selection counts against us also, since we can’t specify which vaccination was actually given.

      With these issue, we can’t see how any clinic and provider in our setting will ever meet this measure. How is this a fair measure to attest to when the requirements for this measure are too far out of reach to attain?
