eCQM guidance needs to clarify what data is gathered for the ED departure date and time performance measure

      Possible incorrect time being reported for ED Throughput Measures which are not consistent with IQR rules: Per eCQM guidance not clear on what defines date and time of departure. Our Vendor is reporting "admission order" date and time as ED discharge date and time, which is not consistent with the reporting of how long the patient is in the ED from arrival to departure. Data reported is skewed to "admission order date and time". This need to be addressed to align the eCQM with IQR guidelines and to meet the intention of the performance measure. This effects both the ED 1 & ED 2 Measures. ED 2 which looks at the ED Admission date and time to ED Discharge date and time. The current guideline allows the reporting of the ED admission order date and time for both data elements.

      Please clarify if the actual ED departure date and time (what is measured and reported through IQR) or the Admission Order date and time is the correct reportable information through eCQM for the data element "ED Discharge Date and Time.

      The IQR reporting of Quality Indicators Guidelines is specific that the ED departure date and time is the last documented date and time the patient left the ED. The eCQM guidance is to measure the ED Discharge Date and Time which does not clarify what is to be reported. Our vendor has mapped the "ED Admission Order" as the "ED Discharge Date and Time" this is inconsistent with the intent of the measure which is to improve performance of Hospital Emergency Department Throughput from ED arrival to ED discharge/departure (ED 1) and from ED admission order to ED discharge (ED 2). If we are measuring ED Arrival to ED Admission Order Date and Time as the ED Discharge Time (ED 1), and for ED 2, we are reporting ED Admission Order Date and Time to "ED Admission Order date and time" as the Discharge Date and Time, we are reporting incorrect data. This is inconsistent with the CMS IQR reporting guidelines, and which is the basis of the eCQM reporting. We are also not reporting the intent of the measure which is to report and improve the ED Throughput times for patient care from arrival to departure AND admission order to departure. Please clarify and provide more guidance to this measure.
