CMS100v4 lists 3 data elements related to Medication Order: Aspirin ingredient specific, the order and the 2 reasons for the order not done in the Data Criteria itemized list section

However there are only 2 substance administration entries in the measure XML, 1 for the patient refusal reason and 1 for the medical reason refusal. I do not find the Medication Order entry that does not contain the the actionNegationInd=true which I assume is what would provide the 3 entries in the Data Criteria section.
In summary, I see 3 entries for this valueset (ordered and 2 reasons) but only see 2 documented in the substance administration entries.
Are we to document the Medication Order if it was ordered?
