Thank you for your inquiry regarding the Medication Order: Aspirin ingredient specific and Medication, Order not done data elements. The data element 'Medication Order: Aspirin ingredient specific' is needed in order to construct the logic to create the negation rationale for the medical or patient reason. Without the 'Medication Order: Aspirin ingredient specific' data element, there is nothing to negate. Therefore, yes, the medication order should be documented if it was ordered.

We hope this has helped to clarify your inquiry. Please let us know if you have additional questions.

QRDA 1 reporting confusion for measure CMS 100 v4

CMS100v4 lists 3 data elements related to Medication Order: Aspirin ingredient specific, the order and the 2 reasons for the order not done in the Data Criteria itemized list section

"Medication, Order: Aspirin ingredient specific" using "Aspirin ingredient specific RXNORM Value Set (2.16.840.1.113762.1.4.1021.3)"

"Medication, Order not done: Medical Reason" using "Medical Reason SNOMEDCT Value Set (2.16.840.1.113883.3.117.1.7.1.473)"

"Medication, Order not done: Patient Refusal" using "Patient Refusal SNOMEDCT Value Set (2.16.840.1.113883.3.117.1.7.1.93)"

However there are only 2 substance administration entries in the measure XML, 1 for the patient refusal reason and 1 for the medical reason refusal. I do not find the Medication Order entry that does not contain the the actionNegationInd=true which I assume is what would provide the 3 entries in the Data Criteria section.
In summary, I see 3 entries for this valueset (ordered and 2 reasons) but only see 2 documented in the substance administration entries.

Are we to document the Medication Order if it was ordered?
