Need understanding of Medication, Active use in SCIP-INF-2 (NQF 0528, CMS172 v4)

      Thank you for your question. This logic is intended to represent that a patient had antibiotics within 24 hours prior to arrival at the hospital, but they did not have any antibiotics after arrival. If this is true, then they will be excluded from the denominator. (See 7th exclusion in the narrative.)
       
      In reviewing this logic as well as the QDM model, the datatype Medication, Active can be used with a "route" attribute. According to the paper chart based specifications, part of the data that needs to be retrieved is whether or not a patient was on any medications within 24 hours prior to hospital arrival. By using Medication, Active we are asking the patient if they have taken any medications within 24 hours prior to arrival and was the medication in IV, PO or IM form. The use of Medication, Administered is a bit different in that it would need to be documented in the record that the medication was administered to the patient, whereas the use of Medication, Active the patient himself would have taken the medication and he could have used one of the routes that is part of the attribute "route," in this case, IV, IM or PO.

      In SCIP-INF-2 (CMS 172 v4), the following logic is used in the denominator exclusions:

      OR:
      AND: "Medication, Active: IV, IM, PO Antimicrobial Medications (route: 'Hospital measures-Route IV, oral or IM')" <= 24 hour(s) starts before start of "Occurrence A of Encounter, Performed: Encounter Inpatient"
      AND NOT: "Medication, Administered: IV, IM, PO Antimicrobial Medications (route: 'Hospital measures-Route IV, oral or IM')" during "Occurrence A of Encounter, Performed: Encounter Inpatient"

      The QDM of Medication, Active is not typically associated with a route of administration. This, combined with the timing constraint of LE 24 before admission and NOT administered during admission, makes me wonder if this should be referencing the Medication, Administered QDM. Please advise. Thank you.
