“FIRST” denotes the first occurrence in the medical record. It is specified this way in order to limit each condition to being counted only once. In order to satisfy the block of logic, at least 3 of the “OR: FIRST…” statements must evaluate as true.

In the above block do we need to take Least recent startdate of at least three or more “OR’ conditions to satisfy above block? How to interprete the above conditions?
In the attached measure spec We have not considered FIRST (Least recent startdate) of each ‘OR’ condition.
