What is the proper place in QRDA Cat I file to indicate Medication Administered is on HOLD

In this line of logic: AND: "Medication, Administered not done: Hospital Measures - Hold" for "Hospital Measures-Aspirin " starts during "Occurrence A of Encounter, Performed: Encounter Inpatient" the "hold" term is part of the value set name. This may be confusing as most of the "not done" logic statements use the standard Patient Refusal or Medical Reason value sets, but this one uses "Hospital Measures-Hold" value set.

      In terms of the reporting, this should be reported just like all other “not done”s.
      1. Use the Medication Administered template (2.16.840.1.113883.10.20.24.3.42)
      2. Set the negationInd to “true”
      3. Use the entryRelationship to link to the Reason template (2.16.840.1.113883.10.20.24.3.88)

    • Incorrect placement of data element could change intent of Medication status

      The EH measures eAMI-2 (CMS100v3 NQF ID 0142) and eAMI-10 (CMS30v4 NQF ID 0639) both have logic looking for "Medication, Administered not done: Hospital Measures - Hold"

      The value set "Hospital Measures - Hold" (2.16.840.1.113883.3.666.5.1145) only has one SNOMED code: 421139008

      We could not find any examples of this condition in the QRDA Cat 1 documentation (CDA Release 2 - QRDA Category I, DSTU Release 2) or from our test Cypress files.

      The only examples found online using this SNOMED code were for CCD and are all using the Medication Status Observation template (2.16.840.1.113883.10.20.1.47); but that template is not included in the QRDA Cat 1 documentation.

      So can you please confirm we should be using the Medication Status Observation template (2.16.840.1.113883.10.20.1.47) when indicating a hold status for Medication, Administered. Or if not, then please explain what template should be used and/or where in the QRDA Category 1 file the "Hold" information should be placed.
