Medication hold is one of several options used to indicate a documented reason for not prescribing a statin medication at discharge. If your EHR or clinical practice does not allow for medications to be put on hold, then this option for the denominator exception will not be available, but other options, including patient refusal, allergies and other medical indications are valid for meeting the denominator exception conditions.

CMS30v5 and CMS100v4 - each has a Value Set for “Medication Hold”. In our clinical application we do not currently allow a medication to be placed on hold. In order to successfully certify to these two measures, do we need to create the option for a medication to be placed on “hold”? Or, will it be acceptable to state that our EHR does not allow medications to be placed on hold?
