      The RxNorm value sets include the RxNorm identifier and the name of the medication concept. Since the value sets contain multiple "types" of codes (concepts) as represented by the RxNorm TTY, the names will vary with regard to the amount of specific drug information represented by the code. NDC codes are not provided in the value sets because they change too frequently. You can get the associated NDCs for many RxNorm concepts by using the applications (and APIs) available from the NLM that give access to the RxNorm database, which includes many (but unfortunately not all) NDCs for each active prescribable medication. I've just checked RxNorm 1364445 using the RxNav application and it does not appear to have any NDCs associated. That means you will need to use your drug information vendor, or supply vendor to help map the NDCs in your system to the appropriate RxNorm code.

      VALUE SETS for Oral Factor Xa Inhibitor for VTE Prophylaxis: RX Norm: 2.16.840.1.113883.3.117.1.7.1.134. There are FIVE individual VALUE SETS under this one RX Norm. The PRIMARY MINEMONIC is listed within each one. When we are trying to match our forumlary to these five Value Sets, how should the meds LINK, by the NDC? example: 1364445 (RXNORM, 2014-01) is 1364445 the NDC? If the Primary Minemonic and the Value Set Name are different, would that create a 'ZERO' on our end for eCQM success? or do the value sets match by NDC alone, regardless of what the NAME display is?
