Weight screening measures not counting 17 year old patients

      CMS and ONC are working to try and address the measure gaps. The challenge is the expectation regarding the transition between providers around the transition to adult medical management as well as the requirements of the adolescent and adult measures. We will try to bridge the gap as much as possible; however, not all patients transitioning to adulthood may be included in a measure during the transition. The expectation from CMS and ONC is that providers would continue to appropriately screen patients regardless of whether they temporarily drop out of the measure because the activity is clinically appropriate.

    • Not accounting for a large population of patients

      Child weight screening (EP 0024) checks for patients < 17, adult weight screening (EP 0421) checks for patients >= 18. I realize these are separate measures, but they have similar intent, and when used together they don't consider 17 year old patients.
