QUESTION: Stage 2 Core #15
Definition of transition of care:
Transition of Care – The movement of a patient from one setting of care (hospital, ambulatory primary care practice, ambulatory, specialty care practice, long-term care, home health, rehabilitation facility) to another. At a minimum this includes all transitions of care and referrals that are ordered by the EP.
Question – Would an order for Physical therapy/Occupational Therapy/Speech Therapy be considered a transition of care? Patients are ‘referred’ to therapy, however a rehabilitation facility is usually inpatient, while most patients go to outpatient therapy. Should the provider include all ‘therapy’ orders in the denominator or does the provider have the flexibility to include or exclude as long as it is consistent across all patients for the entire EHR reporting period?
