      For measure EP_CMS137v2_NQF0004_Ini_Eng_Alc_Drug_Dep, the numerator 1 and 2 are separate in the flow chart but in the spec document both numerators are together with an AND statement.
      Our assumption is that we need to calculate 2 performance rates. Please clarify.
