      There are three measures where a numerator of zero is the preferred outcome, for those measures is a file with a denominator greater than one and a numerator of zero acceptable:

      CMS Measure: 156 - High Risk Medications in the Elderly
      CMS Measure: 75 - Children with Dental Decay
      CMS Measure: 122 - Hemoglobin A1c > 9.0% in Adults
