      QUESTION STAGE 2 CORE 12 PREVENTIVE CARE (PATIENT REMINDERS) 2 OR MORE ENCOUNTERS?: (see attached pdf)

      Question: This is a unique patient measure. The measure requires 2 or more office visits with ‘the’ EP within 24 months before the beginning of the EHR reporting period. Does ‘the’ EP mean that the same EP has to see the patient at least twice during the 24 months in order to meet denominator? My understanding and interpretation is it should be the same provider for both encounters.

      Example: this patient should not meet denominator for either provider, correct?
      • Encounter1#: ‘Provider A see then 1/31/2014’
      • Encounter2#: ‘Provider B see then 6/31/2014’
