      CMS 133 limits the cataract procedure time period to look at only those performed between Jan 1 and Sept 30 of the measurement period because the measure requires a 90 day look back to determine if the numerator action was achieved. Contrary to what was indicated in the question, providers reporting during October 1-December 31, 2014 would actually be reporting on cataract procedures that occurred July 3rd through Sep 30th, per the guidance for this measure. Based on the current measure logic, cataract procedures performed from Oct 1 through Dec 31 of the measurement period will not be reported. In order to allow the full amount of time (90 days) after the cataract procedure, by when the patient should reach visual acuity of 20/40 or better, there needs to be a cut-off date for when the cataract procedure is performed.

      Question CMS133v2:

      The Guidance indicated in specification states “The unit of analysis for this measure is each cataract procedure, including instances where more than one cataract procedure was performed during the measurement period. Only procedures performed during January 1 – September 30 of the reporting period will be considered for this measure, in order to determine if 20/40 or better visual acuity has been achieved within the 90 days following the cataract procedure. Cataract procedures performed during October 1 – December 31 are excluded from the initial patient population.”

      As a result for providers reporting in a 90 days window such as example October 1 – December 29 2014, the provider will have zero results for the measure as no patients will meet cataract procedures performed during January 1 – September 30 of the reporting period since they are only reporting on dates outside of this time frame with the 90 days window selected. Is this correct? The measure should only be looking for cataract procedures performed during January 1 – September 30 regardless which 90 days reporting period the provider has selected. As all Cataract procedures performed during October 1 – December 31 are excluded from the initial patient population. Please confirm.
