CMS provides the following guidance: The intent of this measure is to monitor body mass index (BMI). There needs to be care coordination related to BMI among varying providers, most especially with the Primary Care Provider (PCP).

Reporting accuracy on intent of the measure.
In the Guidance section of the measure spec it states, "this measure may be reported by eligible professionals who perform the quality actions described in the measure based on the services provided at the time of the qualifying visit..." but in multi specialty clinics and academic medical centers who share a patient's medical record, there is no way of filtering the patients data by provider to report on just what a certain provider documented in the patient's health record and it is not feasible to require providers whose professional practice does not require them to perform a BMI to document a reason they are not performing a BMI. Electronic reporting uses all data in the entire record. So, if a patient has been managed by his primary care provider but the "most recent" "Encounter, Performed: BMI Encounter Code Set" in 6 months is an encounter that does not require a BMI to be done then the provider who was adequately managing this patient's BMI would not get credit for the care given to this patient.
