Multiple code systems for discharge disposition

      "HL7 Discharge Disposition" code system has one set of values for discharge statuses. Whereas, the SNOMED has a different set of discharge statuses, though some of them may be common. It is very confusing to see the use of a conflicting terminologies between C-CDA and QRDA for discharge statuses.

      1) Is there a mapping available, somewhere, between the "HL7 Discharge Disposition" codes and SNOMED discharge statuses that we can use readily?

      2) In the case of Patient Expired, SNOMED has a very detailed statuses. But HL7 Discharge Disposition has very few. Are we expected to maintain the mapping between these two code systems for discharge statuses and use the appropriate code system (HL7 Discharge Disposition for C-CDA and SNOMED for QRDA) ?
