How is the compliance validated for HMPC document?

      The HMPC document is designed to assess that all the elements of appropriate asthma plan of care were documented and given to the patient. However, the HQMF and QRDA did not have the technical savvy needed to capture the individual data elements/fields. Therefore, you will not be able to capture these discretely. Instead you will need to incorporate all of these into a document that you give to the patient. If audited, it is expected that you can support the data in the QRDA/eCQM.

      In the Guidance of CMS26v1, it is mentioned as below:

      "The home management plan of care document should be a separate and patient-specific written instruction. The document must be present in the form of an explicit and separate document specific to the patient rather than components or segments of the plan spread across discharge instruction sheets, discharge orders, education sheets, or other instruction sheets."

      "The home management plan of care can only be considered to comply with the criteria outlined in the measure logic if it meets the requirements outlined above and is appropriately filled-out with information specific to the patient."

      As per my understanding of the Spec, the generated QRDA does not include any of the details filled in the HMPC document. But includes only high level information that "Asthma Management Plan" communication is provided to patient.

      How is the verification done for compliance that HMPC document is filled-in for the patient as outlined above?
