Confusing period qualifier for gestational age finding in IPP of CMS9v3

      Thank you for raising this issue. In the next annual update cycle, we plan to remove the <1 day threshold. With this revision, documentation of gestational age >=37 weeks any time during the Inpatient Encounter would suffice for inclusion in the IPP.

      Thank you for your question. The measure specifies the Physical Exam, Finding: Estimated Gestational Age at Birth (result >= 37 week(s))" be determined on the first day of the encounter. “<1 day starts after start of encounter, means that X occurs on the same day (aka Hospital Day 1) as the encounter start, but after the start of the encounter”.

      Consider the following criterion from the Initial Patient Population (IPP) of CMS9v3 (NQF 0480, PC-EBMF, Exclusive Breast Milk Feeding):

      • AND: "Physical Exam, Finding: Estimated Gestational Age at Birth (result >= 37 week(s))" < 1 day(s) starts during "Occurrence A of Encounter, Performed: Encounter Inpatient"

      What is the proper interpretation of "< 1 day(s) starts during"? Period qualifiers such as "< 1 day(s)" make sense when used with temporal operators that contain "before" or "after" (e.g. "< 1 day(s) starts before start", "< 1 day(s) starts after end") but the meaning of a period qualifier with "starts during" is unclear. Should the period qualifier be removed, leaving just "starts during"?

      The same criterion exists in both IPP 1 and IPP 2.
