CMS 149+ when documenting procedure NOT done (pt or other reason) Measure is looking for a LOINC code. LOINC represents a result and if proc was not done there should NOT be a LOINC associated with it.

      'Medical Reason' and 'Patient Reason' are value sets specified in SNOMED-CT and are intended to specify the reason for the 'Risk Category Assessment: Standardized Tools for Assessment of Cognition' and the 'Intervention, Performed: Cognitive Assessment' not being performed, as indicated in the QDM type of 'Risk Category Assessment not done' and 'Intervention, Performed not done'.
      In implementation – there would need a reason from the SNOMED value set for not performing one of the assessments of cognition that are specified in LOINC and SNOMED-CT for a patient to be applied as a denominator exception. As such, the requirement is that a SNOMED-CT code will be reported to meet the denominator exception criteria for these data elements.
